Aetna Spravato™ Coverage Requirements
- Age Requirement:
- You must be 18 years of age or older.
- Diagnosis Requirement — at least one of the following:
- Treatment-Resistant Depression (TRD): A history of inadequate response to two or more antidepressants from different classes.
Major Depressive Disorder (MDD) with suicidal ideation or behavior.
- Additional Required Conditions (Must Meet All)
- Depression must be diagnosed using an approved tool such as the PHQ-9
- Spravato must be used along with an oral antidepressant
- Treatment must be administered under the supervision of a healthcare provider
- Blood pressure must be monitored before and after each dose
- Provider must monitor for sedation and dissociation for at least two hours
- The patient, prescriber, and facility must be enrolled in the REMS program
- Provider must monitor for clinical worsening or signs of suicidal thoughts or behaviors
