Stellate ganglion blocks are not reimbursable for the purposes of treating PTSD. Libertas Mind charges much less than other providers because we want the therapy to be accessible. The cost is $700 without anesthesia/ $850 with anesthesia.  Payment is due at least four days prior to treatment.

Spravato is covered by most insurances.

The charge for the initial appointment for both medical and psych consultations is $225.00. Follow up consults after a procedure is $150. 00

Medical and follow-up consultations are billed through your insurance. You may need to pay a co-pay. Ketamine infusions and Stellate ganglion block for treatment of PTSD symptoms are an out-of-pocket expense.

Ketamine

No. Ketamine infusions augment your current mental health treatments and you continue your prescribed medications. We collaborate with your psychiatrist and/or therapist. It is recommended that you wait at least 3 weeks after changing your medications before starting infusions. Our focus is to provide safe and effective Ketamine infusions, we will not be managing any of your other medications or refill prescriptions.

The “set and setting” are the most important aspects of a good infusion. “Set” refers to the patient’s mindset coming into the infusion. Having an open mind to the experience and avoiding any negative triggers prior to your appointment, will help you have the best outcome. Examples would include avoiding any confrontational conversations with loved ones, watching the news, With that said, 99% of our patients have expressed feeling nervous before their first infusion; this is completely normal!

The effects on ketamine in treating depression can be significantly lengthened with psychotherapy. 

This is so important we won’t offer ketamine without our  patient’s having a mental health provider in addition to ourselves. Let us know if you need a referral for a therapist. 

Yes, Integrative therapy is quintessential in our care plans!

Our patients have special needs and 1:1 coaching helps crystalize treatment goals and obstacles. This is done before, during and after a series of infusions. 

We provide detailed instructions on how to prepare, how long to fast and what meds to wean or stop.

You’ll meet with our therapist or coach prior to the infusion to “set” your therapy on the right path.

Plan to eat something light at least two hours prior to your infusion. 

Benzodiazepines and Lamtorigine are challenging medications should be weaned prior to an infusion.

 

No driving for 12 hours after EVERY infusion. We require all patients to sign a waiver acknowledging that they will not drive for 12 hours following their infusion. You do not need to be accompanied by anyone; but you must have a  ride home and Uber doesn’t work.

Driving under the influence of a controlled substance is illegal and will result in immediate dismissal of care from Innovative Ketamine.

There is no information to suggest that addiction is a side effect from Ketamine infusions for mood and or pain. If you have a history of substance abuse, it does not exclude you from receiving Ketamine infusions. It’s important to disclose this to the doctor so they can develop the right treatment plan for you.

We can allow a guest to be with you with advanced scheduling, however, this is a very personal decision. Some people can be comforting and helpful, whereas others may increase anxiety. If a patient chooses to have a guest in the room with them during their infusion, we ask the guest to simply sit quietly, not interrupting the infusion by talking or moving.

Libertas embraces a team approach to your health.  You are independently evaluated by a medical and psychiatric provider, and carefully observed by an anesthesiologist during you infusion. You are also paired with a Psychotherapist and/or Health coach for behavioral therapy. 

  • Women who are pregnant
  • Patients with uncontrolled hypertension (high blood pressure)
  • Patients with acute cardiovascular disease
  • Patients with recent concussions
  • Any patient who had an unexpected adverse reaction with Ketamine in the past
  • Patients with conditions that cause an increase in their intracranial pressure
  • Patients with schizophrenia

Ketamine given at low doses induces a “dissociative” or psychedelic state where you may feel dizzy, euphoric, or intoxicated. This feeling goes away within 30 minutes of ending the infusion. While the infusion itself may last 40 or 60 minutes, a typical visit lasts around two hours. During the infusion, you will be seated in a private room with a blanket, eye mask, and noise canceling headphones.

SpravatoTM

You administer SPRAVATO™ nasal spray yourself under the supervision of a healthcare professional at a certified SPRAVATO™ treatment center
SPRAVATO™ is taken twice a week for the first four weeks
After the first four weeks, SPRAVATO™ is taken once a week for a month
After this, SPRAVATO™ is usually taken either once a week or once every two weeks.

The nasal spray medication used in SPRAVATO™ is covered by every major insurance provider.  The criteria for authorization is failure of at least 2 antidepressant medications.

Before you take SPRAVATO™, talk to your doctor about your full medical and depression treatment history, including if you have a history of abusing prescription or street drugs or a problem with alcohol.

The most common side effects that can occur during and after taking SpravatoTM include:

  • Feeling disconnected from yourself, your thoughts, feelings and things around you (dissociation)
  • Dizziness
  • Nausea
  • Feeling sleepy (sedation)
  • Spinning sensation
  • Decreased feeling of sensitivity (numbness)
  • Feeling anxious
  • Lack of energy
  • Increased blood pressure
  • Vomiting
  • Feeling drunk
  • Feeling very happy or excited

If these common side effects occur, they usually happen right after taking SpravatoTM and go away the same day.

Because of the possibility of nausea and vomiting, you should avoid eating two hours before, and drinking liquids 30 minutes before, taking SpravatoTM.

No, administered at small doses in a medical setting, there is no evidence that SpravatoTM is addictive. In contrast, those who abuse ketamine recreationally do so at very high doses to induce psycho-active affects.

Yes, clinical experience supports that Spravato can be effective for patients with treatment resistant depression, including those who have already tried ECT and TMS without benefit.

You’ll take SpravatoTM twice a week for the first month. During the second month, you’ll take it once a week. After that, you’ll take it once a week or once every two weeks. You will continue to take your oral antidepressant as prescribed.

In clinical studies, improvement of depressive symptoms was demonstrated at four weeks by SpravatoTM plus an oral antidepressant, compared with placebo nasal spray plus an oral antidepressant. Not all patients will respond to SpravatoTM.

Some patients taking SpravatoTM may experience nausea or vomiting. Because of this, you should avoid eating two hours before, and drinking liquids 30 minutes before, taking SpravatoTM.

SpravatoTM is a new medication for depression (low mood) that is delivered through the nose with a nasal spray device. It represents an exciting step forward in the treatment of depression. It is a valuable addition to the growing list of treatment modalities in psychiatric practice. This treatment is not currently used for other mental health disorders like anxiety, Schizophrenia, PTSD, OCD etc.

Like Ketamin, SpravatoTM is meant for individuals who have treatment resistant depression, which is depression that has failed to respond to standard treatment approaches with medications and therapy. In other words, SpravatoTM is indicated if a patient’s condition has not improved with two or more medication trials. Usually, this is also a requirement by insurance companies in order to have the treatment approved. 

Stellate Ganglion Blocks

Stellate ganglion block is an injection of local anesthetic in the front of the neck. It relieves pain in the head, neck, chest, or arm due to reflex sympathetic dystrophy (sympathetically maintained pain), causalgia (nerve injury), herpes zoster (shingles), or intractable angina. This procedure can also be used to see if a patient’s blood flow has improved after Raynaud’s or CREST treatment.

There are some surgeries that could cause excessive scar tissue or distort the anatomy in such a way that a stellate ganglion block cannot be performed. Thankfully this is rare!

If you have an active infection, such as flu, cold, uncontrolled cough, fever, very high blood pressure, or cannot stop blood thinners, this procedure is not advisable.

 

Risks -however rare – include seizure, collapsed lung, numb arms lasting for hours, temporary weakness or numbness from neck down, nerve damage, and bruising at the injection sire.

Integration refers to specialized behavioral health coaching.  Our patients have special needs and these can only be addressed by 1:1 coaching. So, the answer is, of course! Similar to ketamine, we believe that the “set and setting” pre procedural mindset apply equally well to stellate ganglion blocks.

This is an expected side effect that comes from numbing the stellate ganglion!

Changes that can be expected because of blocking the sympathetic nerves include:

A droopy eyelid, bloodshot eyes, a stuffy nose, and a temperature increase on the injected site.

There may be light discomfort since the doctor needs to press on your neck to locate the injection site. It uses a very small needle. The anesthetic may sting or burn as it goes in. We can offer you an oral sedative prior to the procedure.

The procedure usually takes 5 to 10 minutes to complete. The IV will start after you sign a consent form and after you have had your blood pressure checked. Skin temperature monitors will be placed on both of your hands. The Stellate ganglion blocks procedure is usually done with the patient lying on his back with a sheet rolled up between his shoulder blades. After the neck is cleansed, the doctor will press on the neck to determine the right injection site.

After the injection, your neck may feel tender or bruised. You may also notice a bit of unbalance. If your arms become numb or heavy, you may use a sling until the feeling passes – usually around 4 to 6 hours. Pain medications may also be prescribed.

After the procedure, you will be advised not to drive or operate heavy machinery.

Results can be felt as quickly as 30 minutes following the procedure and can last for years. This immediate reduction in symptoms, along with a long-lasting sense of calm, can also provide a better environment for additional treatment opportunities like psychological counseling.

From an evidence-based clinical perspective- motivational interviewing and positive affirmation provided by health coaches in addition to therapy has been shown to extend the therapeutic effect of ketamine, and stellate ganglion blocks!
